Apple Academic Eligibility Verification

Thank you for shopping with the CU Book Store!

Before we can complete and ship your order, we need you to fill out and submit an Apple Verification of Eligibility form.

We offer two versions of the form, both PDF and Microsoft Word formats:

There are three ways for you to fill out the form and submit it to us:

You may fill out the AppleEligibility.pdf form electronically and press the “Submit by Email” button at the top of the form. This requires you to have Acrobat Reader installed.
If your computer or browser does not work properly with the “Submit by Email” button, you may print the AppleEligibility.pdf or AppleEligibility.doc form, fill it out and fax it to us at 303-492-8329, ATTN: Buffalo Chip.
You may also save a copy of the form, fill out the requested information, save the filled out form, then attach it in an e-mail to technology@cubookstore.com with the subject "Apple Eligibility Form".

If this computer is for an incoming student and you do not have an official ID number and Identikey name (CU Login name), then please include a copy of the Letter of Acceptance.
